Job Description

The Chaplain is responsible for providing pastoral service to the participants and families of InnovAge, or when appropriate, arranging for spiritual care, which includes crisis ministry, counseling, sacraments, worship, education and community or Church coordination.

  • Provides spiritual support and guidance for participants and families at the Day Centers, homes, hospitals, and nursing facilities regardless of faith or lack thereof, is ecumenical under all circumstances and respectful of all faiths.
  • Adheres to the spiritual needs of participants and family members in collaboration with the multi-disciplinary team as requested.
  • Visits and performs counseling/faith service to participants on spiritual care list, recording these contacts in the InnovAge Electronic Medical Records in a timely manner, as well as for those who have requested such services in collaboration with the Director of Spiritual Services.
  • Functions as part of the Palliative care team by providing faith services to participants who are making the transition and their family member(s) and caretaker(s).
  • Plans, prepares, conducts and attends religious services as requested or needed.
  • Contacts loved ones of deceased participants during their bereavement period as deemed appropriate by the

    Chaplain or Director of Spiritual Services.

  • Responds to emergency calls from participants and family members and tries to assist with available resources.
  • Develops relationships with the community to support the InnovAge PACE mission and acts as a resource guide for services offered.
  • Keeps accurate records, e.g. monthly calendar of activities, spiritual care list, mileage log, etc.
  • Provides spiritual support for staff during or following a crisis in collaboration with the Director of Spiritual Services.

REQUIRED

  • Master’s degree in theology from an accredited college/university/Seminary
  • Requires state issued driver’s license, personal transportation, good driving record and auto insurance as required by law
  • 1+ yr working with the frail or elderly

PREFERRED

  • Master’s in divinity
  • Bi-lingual
